We are excited to welcome you to Lake Mary Behavioral and Infusion Center. If you’re a new patient, here’s how to get started:
- Call Office at 407-732-7266 to open a patient chart by providing demographic and insurance information for patient.
- Then the office staff will create a patient portal for you. Follow the link to create your password and security questions.
- Complete Clinical Forms sent to your patient portal – Fill out your new patient forms thoroughly to help us better understand your needs.
- Once completed, Call Office to Schedule Your First Appointment – Once we receive your completed clinical forms, call us to schedule your evaluation. Our team may contact you to schedule your first appointment once we receive the completed forms.
- Meet with Our Providers – Your initial consultation will include a thorough evaluation and discussion of the treatment options best suited for you by a psychiatric expert. Please note that all patients, including those seeking therapy services only are expected to undergo an initial diagnostic evaluation prior to being referred for therapy services.
